PracticeMate is designed to improve the health of every practice’s community across the country, whilst ensuring they stay engaged with you as their regular GP.

PracticeMate is a collaboration between awarding winning general practice Growlife Medical and leading medical IT support provider Health IT. Growlife wanted a way to improve and automate the preventative health prompts for their patients. What started as a BBQ conversation created PracticeMate.

You can now automate what was previously laborious and episodic work, done on monthly or quarterly cycles. Think prompts for health assessments, care plans, missed childhood vaccinations, screening programs and more. Based on any combination of queries of demographics, clinical or billing data, Growlife is able to automatically send specifically targeted communications to patients, at the time the patient becomes eligible.

With Practice Mate, you can define the conditions or cohorts of patients who are eligible for care, or have missed recommended assessments. Patients reaching milestones like a birthday can be prompted to return for care. This includes things like immunisations required at any age, cancer screening prompts at the relevant age, or turning 75 years and requiring a health assessment. With the embedded AI search capability, advanced search, and a suite of easy to implement out-of-the-box care prompts ready to go, you can switch on immediately, or craft customised complex queries to tailor the message to your specific community’s needs.

An initial set of recipes is supplied with PracticeMate. It is anticipated that a general practice would start with these to automate existing reminder systems, then go onto creating more complex custom automations.

The most popular automations to date include:

• • Prompting patients to receive an over 75 health assessment on their birthday
  • Prompting eligible patients for a 45-49 health assessment
  • Prompting childhood vaccinations – removing significant manual effort

More complex examples you could create include:

• • Improve the care for your diabetic patients by sending out an automated notification if they’re late for their blood test
  • complex, multistep processes; for example when a patient returns a high blood sugar result but is not coded to have diabetes, PracticeMate could email the patient with a request to book an appointment. If an appointment is not booked within a certain time, PracticeMate could then send the patient an SMS.

PracticeMate is cloud based and does not require any additional software to be installed on your computers.
